I've worked out of a home office when not traveling for 18 years. It was
very hard at first to not get lazy as you rightly fear. What I did was I forced myself to go out and get coffee every morning when I first started doing it, simulating a commute. Made me get up, get dressed, leave the house, etc. Transitioning from home to work mode. Not sure where you'll locate your home office, but the further away from your main living area the better. Try to avoid working out of a desk in your bedroom where you can roll out of bed in your boxers and never leave the room.
The great news is that it gets very easy and you'll become so much more productive than you ever imagined with less office distractions. Not sure I'll ever be able to work in a cubicle farm again.
